What is the role of a Health Companion (HC)?

  • Health companions are not just here for your health; they’re here for you. They’re your go-to person for everything, ensuring you get the care you need.
  • A Health Companion is a dedicated person who cares for your physical, mental, and social wellbeing. The health companion hand holds you throughout your healthcare journey and will act as a conduit between the individual, the care team, and the doctor. Their other specific roles include:
  • To assist you in taking part in the assessments, keeping a record of your health parameters, and sending reminders for necessary and timely investigations and consultations.

  • Being the go-to person in times of any need- for coordination in lab and diagnostic tests, hospital referrals, nursing/physiotherapy services, medicine delivery, appointment scheduling, etc.
  • The HC is available from 10 am to 5 pm Monday to Friday.
