  • Evaluation: We help you evaluate your journey with Tata MD by constantly tracking your vitals, process of measuring the effectiveness of the care planogram, and making necessary adjustments to achieve your healthcare goals.

  • A Health Companion is a dedicated person who cares for your physical, mental, and social wellbeing. The health companion hand holds you throughout your healthcare journey and will act as a conduit between the individual, the care team, and the doctor. Their other specific roles include:
  • To assist you in taking part in the assessments, keeping a record of your health parameters, and sending reminders for necessary and timely investigations and consultations.

  • Being the go-to person in times of any need- for coordination in lab and diagnostic tests, hospital referrals, nursing/physiotherapy services, medicine delivery, appointment scheduling, etc.
  • The HC is available from 10 am to 5 pm Monday to Friday.
